To shoot grade on manholes using offset survey stakes, first establish a reference point at the desired elevation for the manhole rim. Place offset stakes at a predetermined distance from the manhole location, ensuring they are level with the reference point. Use a level or laser to check the height of the stakes and adjust them as needed to match the reference elevation. Finally, measure the difference in height from the stakes to the manhole to determine the necessary adjustments for grading.
In construction grade staking: blue tops (2x2x6 to 12 inch stakes) mark the level of finish grade; red tops (2x2x6 to 12 inch stakes) mark the level of sub-grade; the color comes from lumber crayons used by the surveyor to ID the stakes by colorizing stake tops;

A concrete form is just a box that holds wet concrete in shape until it sets. They are normally wood but I've seen metal forms a lot too. It has to be removable when the concrete hardens, usually by taking it apart. First, find your grade, that's the level you want the concrete to be at when it's finished. Drive some stakes around the area and suspend strings where the edges will be. Drive larger stakes along the strings. Fasten the boards to the stakes with the top edge on grade. Make sure to support it a lot along the board with additional stakes because concrete is very heavy. Add or remove fill in the form until you have the depth you want. Your ready to pour.
To read a construction survey stake, look for markings that indicate measurements or alignment. Typically, these markings include distance to a reference point, grade elevation, and other relevant information for the construction project. Consult the project's survey plans and coordinate with the site surveyor if there are any uncertainties.
