Check to see if the rubber o-ring is broke.Maybe take it apart and clean it really good with brake cleaner and oil it lightly.
the extractor might be missing or broken The best thing you can do is take your gun to a qualified gunsmith.

I have a pair of these 20ga and 12 ga and the research that I did put the 12ga at right around $600.
the lever that holds the shells in the tube needs tweaking, take the shotgun completely apart and try to load shells in the tube, if they stay then your problem is the lever isnt moving back far enough to let a shell in.
