There are several reasons for hang fires and or miss fires. The most common is due to damp or old powder or moisture in the barrel (oil water) Try a new batch of powder and see if that cures it. Other sources are poor ignition or spark strength, percussion caps get damp or old and loose there spark, or with flintlocks your flint may be dull or dirty or the flash pan may be dirty. next would be the spark path, the nipple may be damaged or clogged or the hole from the nipple to the barrel could be plugged, or in the case of flintlock the flash hole could be plugged. A good cleaning with hot soapy water should clear it up, and check the nipple for any damage or mushrooming and change it out if there is any question.

To prevent a hang-fire for a Percussion Black Powder gun, always snap off a few Caps without powder. Residual oil in the barrel will be pushed to the bottom of the barrel when you load the first round. It is important to burn out this oil. Point the gun at a blade of grass and pop a cap and watch to see if the cap causes it to move. Or insert a small amount of powder in the gun---without a bullet---and fire it to see if the powder ignites. (This may not work on long barrels as the powder may not settle to the bottom of the barrel.

I am going to assume from the nature of the question that you are asking why this is often held as a wise procedure. There are three basic types of malfunction that can occur with ammunition. The first is a "Squib Fire" where the powder does not completely burn, and the bullet fails to leave the barrel. The second is a misfire where the round fails to activate, and one hears only a simple "click"; indicating that the round is a dud. The third type of malfunction is called a "Hang Fire" and is the reason for this procedure. When the first occur "Hang Fires" behave like misfires, the trigger is pulled and nothing happens. Then...a few seconds later...bang! A Hang Fire is the result when a primer fails to immediately ignite the powder, resulting in the round discharging some time after the trigger is pulled rather than within one second. Thus whenever you have a misfire you keep the weapon pointed in a safe direction and wait to be sure it is in fact a miss-fire and not a hang-fire. I have always advised my students to wait a full minute (assuming they are not in combat or what have you, or course.) And then to remove the dud round.
