Pet rabbits do often try to hide when they're feeling deathly ill.

But a sick rabbit doesn't put the effort into escaping a secure enclosure. It shouldn't be possible for a pet rabbit to just "run away": they should be kept in a hutch or pen of some kind. Not only do you want to keep your rabbit from running away - because pet bunnies can't survive in the wild, they aren't adapted to it and they don't have the instincts - but your bunny should be protected from predators, too.

5 years old is not old at all for a pet rabbit: they generally live 8-12 years.

When you have a pet, and you notice a change in behaviour, you should bring it to a vet. A change in behaviour is often a sign of illness. Medical and health care is a responsibility of the pet owner. Bunnies need vets who have special experience and knowledge, because they're so different from cats and dogs that other vets sometimes make deadly errors.
