There are two kinds of easy bird baths to make. I will cover both.
1: Open space bird bath
Birds love ponds, streams, etc. That is why we made this up. Depending on what kinds of birds you are trying to attract you may need a different size of a plant saucer. I got a medium size. Dig a small hole large enough to fit your saucer in it. Put your saucer in it and... voila! To improve your bird habitat add a feeder and a nesting box.
2: Raised bird bath
Get a pot and plant saucer. Flip your pot upside down and glue the saucer to the bottom. Fill it with water. In no time you may see many birds come to take a dip.

I have never heard of a bird making a nest in a bird bath. I believe they only make nests in trees. I wouldn't worry about a bird making a nest in your bird bath. It's more used as a decoration.
