Well, it depends on how much detail you want. Carbon-dating is actually very accurate to within a few thousand years. This sounds like a long time to us, but when you are talking about a fossil that is millions of years old, a few thousand years is nothing. Of course, you can't say that a fossil has been in the ground since 1:27pm, Monday 5th June, 34,050,389 BC, but considering the time scales involved, we can date fossils pretty accurately.

No. It is possible to date a fossil with reasonable certainty. We can generally narrow the focus to at least within 5% of the time away from the present, e.g. If a fossil is around 100 million years old, we can date it to between 95-105 million years old, but the exact year is unknown (which would be absolute certainty). However, we can say with relative certainty that it is within that 95-105 million year old period, so it is reasonably certain how old it is.

How are sedimentary layers used to date fossils?

Stratigraphic layers can be used to determine the age of a fossil in a simple manner. The layers towards the top of an outcrop are younger (Law of Superposition and Law of Original Horizontality) and the layers near the bottom are older. If a fossil is found in a lower layer then that fossil is older than any fossils found in the higher up layers.
