People are hurt and killed by horses by having a lack of knowledge and understanding of how to deal with these large animals. Horses are prey animals and will react to situations that they are unsure of or frighten them. If you are not familiar with the horse then you MUST be very careful as how to act around them.
In order to be safe you must have knowledge of what you are dealing with and that sometimes isn't always 100%.
I have 4 horses and am ALWAYS aware of what they are doing when I am in the barn or corral around them. They sometimes get silly and though they never mean to hurt you intentionally can cause great harm.
There was one time when I opened the barn door a little bit and my one mare wanted to come in and pushed right through pinning me against the opening and the wall. I thought she broke all my ribs, luckily she didn't but I was bruised and hurting pretty bad for weeks. Was it her fault, no not really, I put myself in the situation, she just reacted.

This depends slightly on what you mean by 'they'. Typically though horses have been put down in only a few manners regardless of who was doing it.
1.Lethal injection: The most modern method is lethal injection of Sodium Thiopental or Pentobarbital injected into the vein (intravenously or IV.) Often thought to be the 'most humane' way to put an animal to sleep.
2. Bullet: A bullet typically from a .32, 9mm, or .38 (most commonly used and preferred ammo) ammunition (.22 caliber ammo typically does not carry enough velocity to properly put the horse down.) The death is instantaneously and much faster than lethal injection. This is actually the most humane way to put a horse down. Nowadays regarded as a last resort or even cruel it is a misunderstood method of euthanasia.
3. Captive bolt: Not entirely humane as it typically only stuns the horse which must then be dispatched with a secondary method. The bolt typically only stuns the horse as most people do not know how to effectively use one on horses as they are designed for cattle and sheep.
These are the three most common methods used today, in the past horses would have also been put down in the following ways.
Cutting the throat. ( Typically a secondary way to insure death and complete blood loss)
Sledgehammer. ( Done by some very old time farmers as their only means to put a horse down.)
Electrocution (Done to some racehorses/ show horses to make it appear as though they had a heart attack for insurance fraud purposes.)
