The court can change the custody order and take primary custody away from the mother. The father should stay on top of the situation by filing a motion for contempt of a court order (visitation) and every time she violates the visitation order he should keep filing motions for contempt. She may eventually lose custody.

You do not have the right to deny visitations but if you do, the father can and should request a court hearing to establish his paternity and get visitation rights. He can request custody or joint custody and the court will render a decision. If the child is to remain with the mother the court will set up a child support order and the father can file contempt charges if the mother withholds visitation or violates the visitation order in any other way.

Yes, where else do you expect your children to sleep? I had to have a bed for my child although he/she was living with the other parent so I bet your husband/wife will expect the same from you. You wouldn't expect your children to sleep with you or on the sofa would you? I certainly wouldn't with mine. I'd expect a bed from him/her to sleep in.
The visitation order must be modified by the court having jurisdiction. It can be changed by agreement of the parties. The judge may hear an older child but there is no requirement to do so. Either parent who violates the existing order is in contemp of court.
