Yes, this really can happen. The transmission modulator ruptures and vacuum system system draws out trans fluid into engine oil thru vacuum pump into engine crankcase. Been there, done that. I replaced the modulator valve and dropped engine oil.

The 1989 Mitsubishi transmission fluid tank can be found on the bottom of the transmission. The transmission fluid tank is known as the transmission fluid pan.
When you changed the Radiator did you flush the engine to remove the transmission fluid that the already mixed with your water in the engine block. If not that would explain the presence of the fluid in the coolant holding tank.

Most Likely you have a leak from the transmission cooling tank on your radiator to the antifreeze portion of the radiator. Jeff

You're talking about the old trick of putting a little ATF into the tank to clean out injectors and such. If your engine is EPA2010 or Euro 5/6 compliant, do not do this. That means, if you have a diesel particulate filter on your diesel - you'll clog the DOC face and the DPF by doing this. For an engine prior to the EPA2010 emissions standards, it can be done, but very carefully.

Just drain the fuel tank, blow out the fuel lines, replace the fuel filter, fill the tank with fresh fuel, and it will be fine. You may have to use starter fluid to get it starter and it may run rough for a few minutes but no damage will be done. Putting gasoline in a diesel will destroy a diesel engine, but putting diesel in a gasoline engine will do no permanent damage.
Drain the fuel tank or pump it out. Remove and replace the fuel filter. Use starter fluid to start the engine. No damage will be done and the engine will run fine once all the diesel is removed.
