This is almost an oxy-moron but no, that would defeat the whole purpose of power of attorney. The Principal (or person giving power) must not only sign the power of attorney form but they must have it authorized in front of a notary.

If by "signing off" you mean to end their duties the answer is no. The principal needs to revoke the original Power of Attorney document and then they can execute a new one naming a new attorney in fact. A copy of the revocation should be provided to any facility where the original POA was used in the past along with a copy of the new POA.
