The antagonist in The Fault in Our Stars is cancer.
(An antagonist is not always a person).
John Green the author of the well aclaimed novel "The Fault in Our Stars" riffed on a famous line from Julius Ceaser by William Shakespeare which is: The fault, dear Brutus, is not in our stars but in ourselves that we are underlings.
John Green believed though that many unfortunate people did have faults in their stars (a fault in their fate) so he continued on to write a book about two cancer patients.
This question can be answered in many different ways. There is not an answer because as John Green said: Books belong to their readers.
Although many people would say that it was when Hazel Grace Lancaster first finds out that Augustus Water's cancer has returned.
I'm presuming you mean the quote for which the book was titled?
"The fault, dear Brutus, is not in our stars, but in ourselves."
Spoken by Cassius, from Julius Caesar, by Shakespeare
They both live with their parents in Indianapolis, Indiana.
That question can be interpreted in many different ways- and the answer changes as the novel progresses. Physically he was fighting with his disease- Cancer. But mentally, Augustus Waters was battling with the idea of his death (he always believed that in life, he had to do something big to make it worth anything) and his identity. Because of his young age (seventeen) he had yet to figure out his life and as the novel progressed we see him detiorating slowly until the scene at the gas station where he loses his hold with everything for a moment with Hazel Grace. Until then he had played at being a hero, but from the moment his cancer returned, we saw his downfall, slowly.
Most of the novel is set in the small town Indianapolis (this is where John Green- the author- grew up) but a small portion of the book is set in Amsterdam- which is described as "The city of freedom".
Seventeen year old Hazel Grace Lancaster who was diagnosed with lung cancer (she travels everywhere with an oxygen tank) at thirteen years old, meets cancer survivor Augustus Waters (who only has one leg due to his previous cancer) at a support group where he was attending to support his best friend, Isaac who's vision is taken away due to his Cancer. There they become quick friends and bond over their love over a book: An Imperial Affliction. Hazel Grace has forever been trying to contact the author- Peter Van Houten- and Augustus Waters uses his wish from The Genie Foundation (similar to the Make a Wish Foundation) to fly them both there. There they kiss and spend the night together and after Augustus tells Hazel that his cancer has returned. The rest of the book is spent with the couple detiorating as Augustus' condition becomes worse and he eventually dies. Hazel Grace is heartbroken but his death causes here to wake up and notice everything around her. The book is very philosophical and discusses many different issues.
Augustus Waters is played by Ansel Elgort.
The Fault in Our Stars fits into many different genres: Romance, Tragedy, Adventure, Comedy, Philosophical.
Hazel was diagnosed with Stage 4 Thyroid cancer. She also had metastasis forming in her lungs.
