That is normal.
turn on your ac and see if it comes on then
the engine temperature has to get to 225 degrees Fahrenheit to turn on. If the engine is reaching 230 degrees and it is not turning on the sensor for the engine cooling fan is most likely the culprit. As for the fan operation of the fan with the a/c that is normal
A fan that is connected to the engine will turn whenever the engine is running, an electric fan will only run when the engine is at the proper temperature, some also turn on whenever the AC is on.
The fan comes on and run continuously when the A/C is engaged which is normal. It will also come on to cool the coolant in the radiator to prevent the engine from overheating once the coolant reaches a predetermined temperature.
The fan comes on when the temperature of the engine reaches a set temperature as you say. The fan will also come on and stay on as soon as the A/C is turned on. Other than those 2 methods, you cannot manually turn it on. There is no reason to do so. The fan's job is to keep the engine from going above a set temperature, and nothing more. No need for it to run any other time.
I believe it turns on and off the fan (1 out of 2) that cools the engine according to the temperature it's at. If it doesn't work the engine may not know to turn the fan on and the engine could overheat. Here's a tip. When you turn on your A/C or Heater, both fans come on. I assume your talking about the engine fan, not the cabin fan. The engine fan is designed to come on only when the engine gets hot enough for it. I.E. the fan won't turn on in the dead of winter, or if you just start your car. The fan will turn on whenever the A/C is turned on though, as part of the Air cooling system.

It is designed to free wheel until the engine reaches running tempture. The way you check the fan clutch is make sure the engine is hot, norther words go and drive it a few miles, then come back and leave the engine running, open the hood then stand somewhere where you can see the fan turning. Then turn the engine off and the fan blades should only turn about 1 time as soon as you turn engine off. If the fan spends several times then the fan clutch is bad. Replace fan clutch. Just be sure the engine is hot when you do this test.
You may have a bad ignition pig tail. It is in the steering column behind the ignition switch.
It the A/C is on the fan will run continuously. Turn the A/C off and if the fan still comes on as soon as you turn the ignition on, the thermal relay may be stuck closed.
