Really, there were no generals involved. Generals are the top officers in an army. The top officers in navies are admirals. The Pearl Harbor attack was planned and carried out by the Japanese Navy. The main force behind the decision to make the attack was Admiral Isoroku Yamamoto, who commanded the "Combined Fleet", most of Japan's seaborne ships. Yamamoto did not go on the attack to Hawaii, he remained in Japanese waters. The Task Force which carried out the Pearl Harbor attack was commanded by Admiral Chuichi Nagumo.
No, Hawaii was attacked only once by Japan on December 7, 1941. However, there were multiple waves to the attack, and multiple locations (including Pearl Harbor Naval Base and Hickam Air Force Base) were attacked on the island.
