Keep in mind that families were bigger, which meant women were more frequently pregnant, and thus not having their periods as frequently!

There weren't days before tampons and pads - they've likely been around for as long as women have been (although remember early women would have spent most of their time pregnant or nursing, thus wouldn't menstruate often). Items like natural sponge, cotton, cloth, papyrus, moss, etc. have been used for tampons for thousands of years, rags as pads have been used often too.
