Much the same as anyone you don't know on a personal basis i.e. Dear Pastor - followed by his or her surname.
You can address a pastor in a letter as "Dear Pastor [Last Name]" or "Dear Reverend [Last Name]." It is appropriate to show respect by using the title before their name.

If the Pastor who is the third and not a Junior is the only Pastor then you address him and his wife: Pastor & Mrs. John Doe. If all three were Pastors then you would address the letter as: Pastor John Doe,III & Mrs. John Doe.
Address it as 'Pastor---(the name of the pastor).

They usually use the title of Pastor.
