You would have to get your PUK code from att.com
1st- you log in to your account then click My Services(under the word 'Shop')
2nd- you click my phone/device
3rd- you click unblock SIM and a new window will pop up and show you the code.

it's very simple just call to your service operator and request them for PUK code.

You can't ! The PUK is set by the network - unless you're the legitimate account holder (and can pass their security questions) - the network will NOT give you the PUK.

If the device is saying PIN blocked and is not asking for a PUK code then you will have to purchase a new SIM card. If it is asking for the PUK code then contact your service provider and they can get it fixed for you.
if you write your Sim card code or PUK code incorrectly many times your whole Sim card is blocked!! i suggest you should get new Sim card instead.
You can - but that will also be blocked. The way to unblock your original SIM - is to contact your service provider. Depending on whether this is the first time, they may charge you to unblock it. It is the phone itself that is blocked - not the SIM card.

if you have now got your phone blocked and it is asking for a pin , you either need to go onto the site of your phone Sim card make and receive a PUK code , or ring up your contract providers and get the PUK that way.

The PUK codes are blocked on prepaid phones. You service provider might be able to help you.
if you mean unblock it with the PUK code, no you should just ring up your service provider and they will give you the code. if you have had the sim blocked on purpose (if it was stolen or lost) you should just ring the phone company and they'll unblock it for free
