Generally, they are placed four to five feet on center but if you have you are using a weak bracket or railing, they will not be able to spaced that far apart and still meet the load requirements of the building codes.

Building codes require that handrail be able to withstand a 200 lb concentrated or 50 lb/ft uniform load. If you use a weak, flexible handrail you will need to place your brackets closer together. Cast or lightweight handrail brackets would also keep you from spreading them out too much.
