Sex is what is between your legs, orientation is who you want to sleep with, and gender is between the ears. This is a person with the sex parts of a male but who is female in gender. The human brain is danged hard to change but the body is easier; to bring the two into congruence, when needed, GRS or SRS is performed. You perceive a person who is 'male' and who's gender you assume is male.
The reason you see a person acting like a boy but claims to be a girl can be explained a couple of ways:

That is a transsexual man. The gender term after the word transsexual is always who the person really is, not what their body was at birth. Calling a man born with female parts a woman is hurtful.But like anyone born with a uterus, pregnancy is usually possible. They would have to avoid taking male hormones when trying to get pregnant and during the course of the pregnancy. If they are on hormones, then they can only get pregnant if they can start menstruating.
A woman being transsexual.. Do you mean a Female-to-Male, or Male-to-Female? IS it her body that is female or her brain? Aye that does not matter. Any transsexual person shows signs of being the other gender early in life. At 5-7 years old the person discovers and tries becoming themselves, whether or not "themself" matches their bodies.
