Could be the fuel injection system. You could try one of the FIS additives in your tank and see if it helps. Sometimes adjusting the timing helps smooth the idle. Check to see if your idle RPM is within the recommended range.
Every 5000 miles an air filter should be changed regularly. You can stretch the life out of an air filter by just blowing some of the dirt out.

You will need to remove the gas tank from your 2004 VW Passat in order to change the fuel filter. Remove the gas line. Remove the fuel filter retaining plate. Reverse the process to install the new fuel filter.
