A low MCV could indicate many different things. The most common or prevalent being anemia. There are many different kinds of anemia, and there is no way to diagnose anemia without other laboratory testing. It could be iron deficiency anemia or it could be a range of genetic variations in hemoglobin composition, known collectively as thalassemias.
