neo terminal ileum refers to the 'new' terminal ileum, i.e the 'new' last part of the small bowel.

It would be referred to as neo or new because the original terminal ileum has previously been removed.

If you haven't had the last part of your small bowel removed you would just have a terminal ileum.

If you have had the last part of your small bowel removed, you will have a neo terminal ileum.
