When you smoke your cilia get paralyzed for 90 minutes, that way mucus can not come up. "morning cough" is because your cilia are not paralyzed while you sleep, therefore all that mucus is trying to get coughed up when you wake up. Most people just start up the day with yet more smoking.

That is a very good question! Smoking kills the cilia from your most of the tracheobronchial tree. Ciliated epithelium is replaced by stratified squamous epithelium. This adversely affect the function of the tracheobronchial tree. The foreign particles are not thrown out side and may cause pneumoconioses. You get repeated bouts of acute bronchitis, leading to chronic bronchitis. There is stagnation of the carcinogens leading to lung cancer, eventually.
