If you are afraid of something so much that the emotion overwhelms your rational circuitry, one strategy you brain uses is to eroticize the thing so that you are flooded with feel-good hormones to cool you off. It is like a nuclear reactor being flooded with water to prevent melt-down. The fetish is like the radioactive vapor that the reactor releases - a byproduct of the emergency action with significantly complicated potential after-effects.
Fetishes are sometimes the result of a big trauma, sometimes the result of repeated absorption of a small message, sometime the result of a small something that just hit you in the right way (as hard rocks or gems can shatter if hit at precisely the right point under precisely the right conditions.)
From there, the process becomes pavlovian. Because you are getting a libidinous reward for engaging with this fear, you do so more often and in increasing doses and facing it begins to dull the fear reaction itself. Eventually you have mastered the fear and it no longer threatens to shut down your brain. But a consequence is that you now associate that activity with sexual stimulation. This is a fetish.
In the case of a cuckold fetish, the fears you are eroticizing are of being rejected or found wanting in the search for mates and love. This may mean being rejected by people you are attracted to, or betrayed by people you have feelings for, or a generalized sense of inadequacy in the gender role society expects from you.
