The activation gates of voltage-gated Na+ channels open, and Na+ diffuses into the cytoplasm.
Voltage-gated Na+ channels change shape, and their activation gates open.
The "Threshold" just means the point at which something occurs, such as sound. The threshold of a sound would be the volume at which you would first start to hear the sound.

Depolarization occurs when a stimulus opens sodium channels which allow more sodium to go into the membrane making it less negative and more positive (toward reaching threshold). An action potential can only occur once the membrane reaches threshold which means it has reached the level needed through depolarization. An action potential is a brief reversal in polarity of the membrane making the inside more positive and the outside more negative, the reverse occurs again once the membrane reaches resting potential.

Calcium (Ca2+) channels open on the synaptic end bulb in response to depolarization (from the action potential), inducing exocytosis of synaptosomes containing neurotransmitter, resulting in neurotransmitter being released into the synaptic cleft...further propagating the signal to the next neuron or set of neurons.
Sodium channels. A neuron's membrane potential may depolarize for many reasons (neurotransmitters, mechanical deflection, electrical synapse, etc). When that membrane depolarizes to the point of its threshold of activation, then voltage gated channels open up an allow an influx of sodium into the cell. This rapidly depolarizes the cell's membrane, causing that upward peak or rising phase to occur.
The activation gates of voltage-gated Na+ channels open, and Na+ diffuses into the cytoplasm.
A response is always triggered by a stimulus. One example is in a neuron (nerve cell). Once a sensory stimulus is witnessed (a stimulus from one of the 5 senses), the cell hits an action potential, and the response is carried out. Simply put: A human eye is closed. Once the eye opens thousands of sensors in the eye pick up the photons bouncing off all objects (stimulus), and the human is able to see (response).

The stimulus causes an "Action potential" which is a nerve signal to the brain to indicate whatever happened. This is an all or none response, but the intensity of pain is given by the frequency of these signals. The stimulus can be very large and the larger it is, the more frequent the Action Potentials.THE SCIENCE:* Voltage gated Na+ channels are opened when the charge within the cell moves past the threshold point - depolarization, causing an AP (once it's past the threshold point, it's an all or none response regardless of the strength of the stimuli and the same signal will be sent to the brain)* Na+ rushes in and another voltage gated channel (this time K+ channel) opens when the charge becomes more positive * K+ channels allow K+ to move out of the cell (down its concentration gradient) repolarizing the cell. (the charge will become more negative because K+ is a positively charged ion)* These K+ channels are slow to close and during this time is called the refractory period (where an action potential cannot occur again) * There is an absolute refractory period where another AP cannot be generated at all and a relative refractory period where it can * The the stronger the stimuli, the sooner within this refractory period another AP can be generated. Therefore, if the stimuli is strong, the APs will be more frequent, indicating a higer intensity of pain.

A relay is an electrical device that opens or closes in response to a small electrical current or voltage. This makes or breaks the connection in another device.
