While the gap between the two used to be relatively large, that has long since closed. To get into either, you will need a strong GPA, MCAT score, solid letters of recommendation, and excellent extracurricular activities for your resume as well as honed interviewing skills. On top of this you need the willpower necessary to complete the entire course of medical school. After medical school, you apply for residency programs which will then allow you to carry a specialty; even after this some opt for a fellowship which results in further specialization in a given field.
