This year (2014) it will be the month of July. Last year it was in July-August. Ramadan is according to the Islamic calendar but also by moon sighting so it usually moves every year on when exactly it starts.

The Gregorian and Hijri calendar are almost the same. The Hijri calendar is followed by the phases of the moon, while the Gregorian is known for 30/31 days in a month except February. So whenever Ramadan falls in a Hijri calendar, than just see what day that is out of the Hijri calendar.

The Islamic calendar is about 11 days shorter than the Gregorian calendar, so Ramadan, or any Islamic month for that matter, will not always be in the same month in the Gregorian calendar. According to the Wikipedia, in 2011 it is from August 1 to August 30.

Yes. It advances 11-12 days on the Western calendar every year (i.e. It went from June 28-July 28 in 2014 and will go from June 17-July 17, 11 days earlier).
