Refers to the black death and the ring of raised inflamed skin from infected fleas carried by rats. People thought the illness and death from the paige was due to smelly unclean air so to avoid becoming ill small posies of sented flowers were carried to avoid the putrid stench. A tissue a tissue we all fall down, or words to that effect. Sneezy, general symptoms of the black death. Fall down... Dead. Bodies were collected in carts in vast numbers as in cities the Death Rate was extremely high, not all were completely dead when piled high on the carts with other rotting corpses. I would say that's pretty morbid for children to sing about.
