We don't know exactly when the gladiators stared fighting, but historians think it was somewhere in the Italian district of Campania. In Rome, itself, the first gladiatorial combat was in 246 B. It was held as a funeral "mundus" or duty by Marcus and Decius Brutus in honor of their deceased father. It was held in the Forum Boarium and three pairs of slaves fought to the death.

They did not. Gladiators were typically male prisoners forced to fight for the entertainment of a higher entity. women gladiators did exist. But they were not very common. It's not that they didn't exist they just didn't fight as much as men gladiators. And during gladiator games there were often special matches. That might feature women gladiators.

The first recorded incident of females in gladiatorial combat is during the reign of the emperor Nero. He made aristocratic women combat each other as a joke. As far as "real" female gladiators, there were very few and usually as a diversion or novelty and not as serious combat as was the case with the men.

We don't know when the custom of gladiatorial combat first started. It is assumed that it began in the Italian area of Campania in celebration or remembrance of a battle. However we do know when they first appeared in Rome and that was in 246 BC. It was a funeral "munus" or duty given for Junius Brutus by his sons, Marcus an Decius. Three pair of slaves fought in the Forum Boararum.

The first recorded gladiatorial fight in Rome was in 264 BC when the sons of Brutus Pera hired three gladiator pairs to fight to the death in the funeral of their father. This was included as part of the munus (plural: munera), a commemorative duty owed the deities of a dead ancestor by his descendants. However, gladiatorial fights were older than this. The Romans imported them from Campania the region around Naples, to the south of Rome. The earliest known gladiatorial schools were in this area. Ancient Roman historian Livy said that the first gladiatorial fights were held in 310 BC by the Campanians to celebrate of their victory over the neighbouring Samnites during the Second Samnite War. However, they might be even older than this.

The gladiatorial fights became part of public entertainments later. This was started by Julius Caesar in the second half of the first century BC. At this point they stopped being fights to the death.
