Most people measure the penis when it is erect because temperature and emotion can effect the length of a flaccid (soft, limp or unerect) penis. Usually a rule (or ruler if you wish) is placed along the upper side of the penis with one end of the rule/ruler touching the pubic area and the measurement at the tip of the penis is then read off on the rule/ruler.
