I actually have the same question the same van. I do not have the part number but I learned it's a dealer-only item and it's not cheap: . I just ran out from the office to pick it up. its small but expensive and they tell me its going to bolted to the frame 'somewhere' for grounding. I now suspect its going to be between the 2 fans on the bottom of the car. its about 2" square with 1 bolt on each side and a receiver for a 4 prong plug. I will try to locate and replace this evening.
Actually, the relay is located on the driver's side rail, directly below the air filter components. It's not easy to get at, but I've done it from below. It would probably be easier to access from above with the air assembly removed. Be careful. I've read on many forums that these relays have failed quite a bit in the past due to getting too hot. They are
supposed to dissipate the heat through direct contact with the frame (there is a heat sync at the bottom of the relay). Ensure that the area it is being attached to is clean and that it is firmly attached.
I have a 1996 Caravan My relay is below the headlight assembly drivers side. remove the headlight assembly (3 nuts in the rear & 1 bolt on top) you should see a black cover (remove phillips screw on the right) remove cover you should see the relay disconnect plug, remove 2 screws remove relay.

The cooling fan relay is under the hood on the drivers side in the fuse/relay box. The position of this relay is indicated on the underside of the lid for the compartment. The power relay for the cooling or the radiator fan is on the driver side front hood. You will have to take out the driver side head light and you will have to remove the front grill also. The power relay is mounted on the frame when you remove the grill. My van is 2006 Dodge Grand Caravan SXTt.
It would be in the Power Distribution Center under the hood, usually on the driver's side.

The cooling fan relay is located below the battery on the side of the chassis. It is held by 2 small I think 8mm bolts. It is best to use a socket to remove it. You will need to remove the air box first.
