It was both the Montague's and Capulet's faults for being so disagreeable and not coming to a compromise. This caused Juliet and Romeo to go behind their parents backs and get married and such. It is also Romeo's fault as well, because he wouldn't have been banished if he didn't kill Tybalt on a whim. And, if Juliet had just told her parents what was going on instead of going along with Friar Laurence's sketchy plan she could have atleast been kicked out on the street and move to Mantua unmarried to Paris. It's many peoples' fault.

It is difficult to assign blame to any single individual, as it was a combination of circumstances and people that ultimately led to their demise.
I do not believe there is one person to blame for Romeo and Juliet's death, because everyones decisions played a role in their suicides. If you want to get really technical, then you could blame the servant at the very beginning. Because he was illiterate, he had to ask Romeo and Benvolio to read off the names on the list that he was suppose to send the invites to for the Capulet's party. This is when Romeo hears that Rosaline's going to this party, and decides to crash it; however, he ends up seeing Juliet and falling in love with her... the rest of the story plays out after that. But, if I were to blame someone, it would have to be Romeo, because he acted irrationally in love throughout the whole play (even more so than Juliet)
Well, Paris insisted on fighting Romeo. Romeo was only defending himself. So on the surface Romeo actually put the knife in, but only because Paris forced him to, and the reasons why he did that are so complex and involve so many people that it's pointless to go into it unless you have an afternoon to kill.
Juliet is to blame for the nurse's bad back because she sent the nurse out for an errand early on a Monday morning and then didn't give her a back rub when she got back. Juliet is to blame for her father's high blood pressure, because she resisted his plans to marry Paris.
Basically it's easy to blame anyone for anything. If you get really good at it you can be a litigation lawyer.
