In the 2013 episode "The Name of the Doctor" a previously unknown incarnation is introduced, played by John Hurt. Although he is technically a 12th Doctor we do not yet know where he actually fits into the numbering, if at all. Smith will be leaving the series with the 2013 Christmas special, at which point another actor will join to play either the Twelfth or Thirteenth Doctor (depending on what is determined for the Hurt incarnation).
Outside the TV series additional actors have played the Doctor in productions that are not considered part of the continuity (canon) of the TV series. This list includes Peter Cushing, who played "Dr. Who" in two 1960s movies remaking TV episodes; Trevor Martin, who starred in a popular stage play; Rowan Atkinson, Richard E Grant, Jim Broadbent, Hugh Grant and Joanna Lumley, who all played the Doctor in a Comic Relief skit called Curse of Fatal Death in 1999; and Richard Grant also played another version of the Doctor in a 2003 animated special made for the Internet titled Scream of the Shalka. Additional actors have played the role for audio dramas, including David Warner and Arabella Weir.
Mention should also be made of The Valeyard, a character featured in the 14-episode story The Trial of a Time Lord played by Michael Jayston who is revealed to be a future incarnation of the Doctor.
But of course if you meant who was a doctor (as in a medical doctor) in Doctor Who, there have been a number of those characters too, most notably Dr. Harry Sullivan, and Martha Jones, who is initially training to be a doctor when she joins the Doctor, but later gets her doctorate and joins UNIT.
Sylvestor Mcoy was still the doctor in 1995. Paul mcgaun wasn't the doctor until 1996. (but there was no doctor who on TV in 1995)

Doctor Who wasn't on the air in 1990. It went into hiatus in 1989 with Sylvester McCoy as the Doctor at the time. During the 1990s the only Doctor was Paul McGann in a 1996 television movie. After that the next Doctor wasn't until Christopher Eccleston in 2005.
