If you mean a third guitar player it was because they wanted Adrian back but didn't want Janick to leave.
Steve Harris, the band's bass player after watching the movie "The Man in the Iron Mask" was inspired by the torturing device called Iron Maiden which had a connection in the movie. An iron maiden is an iron cabinet with front doors on hinges. The doors and sometimes the back part and walls are covered with spikes. A person would be put in there for torturing purposes. As the doors would close, the person would be pierced. Many times with a fatal ending.

Right now, there are no Guitar Hero games in development, so once they start making games again, there may be one. Iron Maiden has been in a few other Guitar Hero games though: - The Trooper (Guitar Hero II) - The Number of the Beast (Guitar Hero III) - 2 Minutes to Midnight (Guitar Hero 5)
Jimi Hendrix is probably slightly more widely known than Iron Maiden; even though Iron Maiden had a longer career, Hendrix changed guitar playing forever. That pretty much explains it.
