This is called paraphimosis and is a potential medical emergency - it can result in difficulty urinating and a host of secondary conditions including kidney trouble.

Treatment starts conservative - a veterinarian will typically use a hypertonic solution to reduce the swelling (edema) in the exposed penis then manually reduce it back into the preputial orifice. If this doesn't work, the dog will need surgery to remove the exposed portion of the penis.

This is called paraphimosis and is a potential medical emergency. In this position, urination can be compromised, leading to retained urine, uremia and kidney trouble.

You should take your dog to a veterinarian today to have this resolved. Medical treatment consists of reducing the edema in the trapped penile tissue (usually with hypertonic solutions) and manual reduction. If this does not work, surgical reduction to remove the trapped tissue is typically necessary.

It isn't out "too far", because it can't be. If your dog's knot has swollen outside of the sheath, just give it a little time and it will fix itself.
