If you have changed all the Plugs, wires and coil pak then look at the fuel injector on that cylinder. Might be a good idea to replace them all. Should clear it up. My 99 B3000 had the same problem. The key was my fuel comsumption. As long as I held the RPM's above 2000, it ran fine. Ideling was a little shakey.
It can be a lot of things, more than likely a cylinder is misfiring, but you will need to have a computer hooked up to it to be sure. It could be something simple like a wire.
Maybe misfiring on cylinder 6. Or 4. Take the car to autozone, they scan codes for free!

The answer is on page 11 in the owner's manual ( "Light is blinking" ). Engine is misfiring, bring it to a garage as soon as possible.
