The Congressional Medal of Honor was established by congress in July of 1862, and signed into law by Abraham Lincoln. The bill to establish the medal was introduced by Senator Henry Wilson of Massachusetts in February 1862. This was actually the second Medal of Honor bill Lincoln had signed, for in December 1861 Senator James W. Grimes of Iowa introduced a bill to authorize a Medal of Honor for the navy. The highest US Military Medal is the Medal of Honor (Each US Armed Service has their own). The highest medals awarded by the US Government are the US Congressional Gold Medal, and the Presidential Medal of Freedom. They are of equal rank.

I believe it was Buffalo Bill Cody, who was awarded the Medal of Honor as an Indian Scout for the US Army in the late 1800's. FYI, it wasn't a "personal" thing. Hundreds of people were removed as recipients of the medal in a review done in 1917. Because the MOH was the only medal the US awarded (til the very end of WWI), many awards were felt to have been awarded carelessly, and were ordered surrendered by the recipient.
