Check with your insurance company before any thing is done.
If the doctor orders nursing and physical therapy in the home and your policy covers that then it is good. The in care facility has to be approved by your insurance company prior to going there. Always check with your insurance company, they are more than happy to help ahead of time. The doctors office usually has someone that does the surgery scheduling & they do alot of the communication and getting approval ahead of time. Don't forget to talk with the hospital discharge planner regarding where to go following surgery, alot depends on what assistance you have at home.
