Any baptism that is performed with water with the words, "I baptize you, N. in the name of the Father, and of the Son, and of the Holy Spirit" in which the water actually runs over the body (sprinkling doesn't count) and with the person administering it intending to do what the Church does, is valid. If any of those are missing, the priest should probably perform a conditional baptism before the person can receive any other sacraments.
