I actually was wondering the same thing. I have a dog who is 11 months old and in her first heat. I called the vet because I was worried she may be pregnant when I noticed her nipples were swollen. I called my vet and they said it is normal during heat.
If the dog is an intact female, who was recently "in heat" and had the opportunity to mate, she may be pregnant. If she hasn't been in heat recently, or didn't have the opportunity to mate, she may be going through a false pregnancy. If the dog is a spayed female, or a male, the enlarged nipples can be a sign of disease, so a trip to the Vet is in order.
If your dog's nipples are getting bigger and it is a female it probably is having puppies soon. So watch over your dog and if they keep getting bigger, and bigger she would most likely be having puppies.
