No. Most airlines use a "Hub" system, which means that all the nonstop flights are either to or from a "Hub", and then you need to change planes and take another nonstop flight from the Hub to your destination. Sometimes you even have to change planes 3 or 4 times to get where you're going. For example, American Airlines hubs are primarily Dallas/Fort Worth (DFW), Chicago's Ohare (ORD), Raleigh/Durham (RDU) and St.Louis (STL). Also some others can be considered hubs, like Los Angeles (LAX), New York's JFK, and Miami (MIA). You're likely to change planes in one of these hubs, if you fly American Airlines. But this is true on most airlines, although the hub cities will be different. You're not going to find a nonstop flight unless you happen to be flying to or from one of the airline's hub cities.

Depends mostly on the airline and where you're going in Hawaii If you're going Delta... many times they will route you through Atlanta for their non-stop flight to Honolulu. If you're going Continental to Honolulu, you could catch their nonstop out of Houston. Generally most flights depart non stop out of Los Angeles or San Francisco on most airlines... but like those mentioned above, it really depends on where you're going. Hawaiian Airlines had Vegas service... not sure if they still do. Northwest had service out of Minneapolis and I believe there might be a non stop out of Chicago to Maui (ogg)
