Yes, tragically there have been a number of fatal accidents on amusement park rides often some of the milder ones , such as centrifugal force rides- a Mother and child died of injuries being tossed out of a Himallaya ride at Coney Isle in l996. These wind-up ( centrifugal force) rides have heavy G- forces and bars can let go. oddly the more obviously dangerous-looking rides such as Roller coasters have heavy-duty bars and safety devices and accidents are much rarer.
