Most of the words in the Urban Dictionary are real words, but used for non traditional meanings. Some real words are combined to make new uses for the words. A few words in the Urban Dictionary are newly made up words, but not as many as you would expect. The new uses for real words or the newly made up words usually are from a specific geographic area, and used for a limited time for their new purpose. Occasionally, every once in a while, one of the words or terms come into such common usage that they eventually make it into the standard dictionaries. For example, the slang use of 'cool' and 'hot' have made their way into dictionary definitions.

Any word may be considered 'real' when enough people use it. This is why all language dictionaries add new words, or altered forms, meanings or pronunciation of existing entries, to each new publication.

Online dictionaries update their entries immediately they are certain a change, or new entry, is valid.

The form of any language - its use, spelling, pronunciation, and so on - is quite literally decided by the people speaking the base language.

So, words appearing in the Urban Dictionary are 'real' words provided the word becomes, or has become, sufficiently popular to justify that designation.

All concientiously-maintained dictionaries add, delete, or alter entries from each new publication of their short versions - compact, or condensed, dictionaries - according to usage.

Large and authoritative publishers always maintain comprehensive dictionaries which include words no longer in use, or words whose meanings or other attributes have changed since the last publication; some of these words will be dropped from shorter versions of a dictionary.

Urban Dictionary contains slang and informal words used in everyday language, so the words are real in the sense that they are used in informal speech and writing. However, they may not necessarily be recognized or accepted as standard English in formal contexts.
