Never. Hair inside a riding safety helmet causes it to not fit properly. An ill fitting helmet can result in death if the rider falls or is kicked. Hair should be tied back under the helmet as soon as it is long enough to be caught up in a hair tie. The age is not important, the length of hair is.
When horseback riding on a public road, you should always wear a helmet, regardless of age. It's just the safer thing to do, as you cannot predict when your horse will spook or shy at something. If you want an actual age, though, I'd say probably 21.

Most children start horseback riding lessons at age seven - before that, most children have less control over their bodies, so will have difficulty staying on, or knowing how to handle the horse, and the instructor may have to take extra care with her.
