You need to do 3 Fundamental things which are as follows; 1. Assess the bleeding and whether injury human/animal can be elevated 2. Clean wound with sterile antibacterial substance ie, warm water hibi scrub mild disinfectant 3. Ensuring the wound will stay clean and protected bandaged, rested etc

This is a difficult question in that almost no information is given. A large wound to a leg in my book would likely be 20-30cm long and quite deep, open perhaps to a point of 3cm. However, I've also see people describe large wounds as 2cm in diameter. Naturally, the treatment for either of these will vary considerably. Another point of variance is what caused the wound? Gunshots, lacerations, stasis injuries and burns (just a few) all will have considerably different treatment approaches. This then in the most generalized way: # Control bleeding. Nothing else matters if your patient bleeds out. # Control shock. Maintain patient temperature and comfortable breathing. # Bandage the wound to protect it, and transport. # If there has been significant blood loss, replacement is a requirement. Treatment of hypovolemia is beyond the scope of this response except to say you may need to replace the missing blood. This too is best performed at a hospital or a well-equipped ambulance. # If you can't get to a hospital, you'll need to consider removing any foreign material from the wound. This means that you need to know if what you're removing is near blood vessels, neural tissue, connective tissue and/or bone, so you can get a good picture of what you're going to do and what's going to happen when you do it. Removing an intact bullet from the leg, only to open up the femoral artery doesn't help he patient and swaps a serious injury for a grave one. # #* If you don't remove foreign bodies, infection is likley. #* If you remove foreign bodies without an idea of the locale, you're literally operating in the dark. #* You should only touch foreign bodies if you can't get to emergency care in time, AND you know what you're about to do. #* The decision must be yours. Again -- if you can get to a hospital, do it. #* Better to leave a foreign body in place and treat for infection than to kill the patient outright. #* For really large wounds, I would advise systemic pain meds first (if patient is conscious and no threat to respiration is perceived), and then flushing the wound with normal saline or sterile water. For really large wounds (my definition), hydrogen peroxide is too damaging. Likewise, introducing large quantities of any disinfectant into the patient's system (which can happen in large wounds) is risky. So water is all you can use. #* Pack the wound with clean gauze and wrap. Wound check often. Note that suturing really large wounds is not easy (especially as it requires a high degree of antisepsis/sterility and may require more than one layer of sutures), and unless you KNOW you can suture the wound closed and it's safe to do so -- AND you have experience in this -- don't do it. Pack the wound open (again, this depends on wound size). Note that wounds packed open heal from the bottom and take a LONG time to do it. # If no major foreign bodies are expected or detected in the wound, you can try to disinfect it. In the field, this is a painful and dangerous process, which will increase the likelihood of shock for your patient. # If you have it, consider a prophylactic course course of wide spectrum antibiotics. Adminstration of antibiotics is also beyond the scope of this article -- make sure you know what you're doing before starting. #* Fever #* excessive or increase amounts of pus in your wound check #* a putrid or sweet smell coming from the wound #* blackening tissue #* Decrease in patient level of consciousness and response. #* dark lines radiating from the wound. #* Uncontrollable bleeding. #* Reduction of pulse distal from the wound -- i.e. farther away from the victim's heart than from the wound. #* ANY OF THESE SYMPTOMS means you're in serious, potentially lethal territory and need a hospital. We've also gone beyond the scope of this article again. # Monitor the patient frequently until the wound closes. You need to look for: In summary, without knowing what "large" really means, what you've read is a guess. There's a saying in medicine: Primum non nocere. It means, "First, do no harm." Please keep this in mind when you address this problem. I'd urge you to consider getting a "large" wound to a doctor, if only for evaluation.

stop the blood with a cotton ball. then, disinfect it with hydrogen peroxide or rubbing alcohol. finally, apply a band-aid.
