It is realized when everything "clicks" and you trials how to run your life properly without desire or suffering. This is personal achievement needing no god to help you.

Death after enlightenment is the last and you may enter Nirvana a state of selfless existence where you blend seamlessly with everything. or you may chose to reenter the cycle of death and rebirth to aid others in their search for enlightenment.
