Metronidazole is a special kind of antibiotic that is aimed at so called anaerobic bacteria. These bacteria do not need oxygen for growth. When you are suffering from sinusitis, most likely other bacteria are involved. So therapy with metronidazole as a single agent can definitely not be recommended. The antibiotics of choice are amoxycillin (or ampicillin) i.g. combined with clavulanic acid. Other antibiotics that should work well are doxycyclin or the macrolides like erythromycin or azithromycin
