The easiest way to train cats, [or ANY animal really], to do something they dislike is to "association" train them. Train them to associate a big wonderful treat with going to the doctor. For example, reward them for being in the car on the way there, even if they didn't behave. They will understand that car ride = treat When you get to the door of the doctor's office, give them a treat. When the doctor comes in to look at the cat, give them a treat. When the doctor holds up a bottle of eye drops, give them a treat first. After the doctor has put in the drops, give them EXTRA treats. Basically, the cat has to associate LOTS of happy fun tasty treats with the whole idea of a doctor visit, so that the little moment of unhappiness is overshadowed :D
