Added: Under the ADA Code of Ethics I assume that they may not withhold treatment from you for THAT reason alone. However, they could ask (or require) you to sign a medical release form first, releasing them from the liability of your decision to not follow their prescribed course of treatment.
Dentists (any Doctor actually) is an independent businessperson, and they are NOT REQUIRED by law to treat you except for immediately life threatening emergency situations.
