Very contraversial question. I think pharmacists should not make the decision to dispense or withhold anything based on personal convictions alone, unless it is made publicly clear that the pharmacy reserves the right to do so (even then, I'd question the legality of it). Like other professionals, pharmacists need to check their personal ethics and morals at the door when they come in for work.
In certain states, both pharmacists and advanced nurse practitioners may prescribe medications relating to specific ailments from a short formulary of medications. This is approved on a state-by-state basis, as allowed by the State Board of Pharmacy and Attorney General of those states, as in Florida. In most other states, pharmacists, especially possessing a Doctorate of Pharmacy degree, have the de facto ability and opportunity in hospitals to assist physicians in prescribing many medications, especially antibiotics.
A pharmacist can refuse to fill a certain prescription. BUT in most cases he/she will have to return the original prescription to you. (unless they feel this is suspicious such as a forgery). If this happens, contact your Doctor Who wrote the prescription.
Absolutely! If a physician feels a medication or medications is not an appropriate treatment for a condition, he/she can refuse to write a prescription. The doctor must act on behave of what he/she feels is in the best interest of the patient.

A Pharmacist
Not normally, although in an emergency the pharmacist can use their professional judgement to dispense, usually a single dose, of a medication if they see fit.
Dispense medicine according to doctors prescription
A pharmacist can be a pharmacologist, but not vice versa. Pharmacologists are researchers who typically work in a laboratory setting, whereas pharmacists actually dispense medicine in a pharmacy (pharmacologists are not qualified to do this). Basically, pharmacologists research the drugs, pharmacists dispense them.
Of course. That is if a prescription has been authorized and duly signed by a medical professional.

In the United States, pharmacists require a doctor's prescription to dispense medicine. In Mexico, the pharmacist can decide what medicine to give without the patient seeing a doctor first.
