It's a rare occasion, however it has happened. Sometimes (although very rarely) there is a breakdown in communication. The student goes to the ceremony and finds out later that he/she is missing a requirement. Then, everyone starts pointing the fingers, It's very rare as I stated, but it is not an impossibility. Always double check.
